Who can avail:
Existing Credit Card Holders and
Non-Credit Card Holders
Age between 18 - 65 years old
Income at least P150,000 per annum or P12,500/month
Have at least 1 telephone line
Employed for at least one (1) year or Self-employed
Existing cardholder of any major credit card company
Applicants must submit the following:
Two (2) Valid ID's (Company ID, TIN. SSS, Driver's License, PRC ID)
Proof of Billing (Meralco, PLDT, MWSS, etc.)
Proof of Income (ITR or Form 2307 for employed, 1701 for self-employed
Fill up the Form and send it back together
with the required documents.
